ADDITIONAL FEES:

 

There is an additional $25 dollar fee for completion of forms for use outside of the patient's medical record, including:

  • employment or school absence notes

  • medical referral notes

 

***Orthopedic Assesments and Medical Reports cost to be determined on a case by case basis.
